Co-ordinated care planning

Co-ordinated care planning is a module to simplify the co-operation with external care providers regarding patients who have finished their treatment in the hospital. Co-ordinated care planning monitors and manages the exchange of information between the different care providers in a region.

Monitoring system

The module monitors all admitted patients to ensure that there is no risk of missing confirmation to the referring institution or the initiation of treatment. The monitoring system is flexible and can be disabled for wards and units which do not participate in regional care planning, such as intensive care units.

Overview

An overview presents status information from the regional care planning module. The overview even shows summary information from the medical records to help the user in the planning of patient care after hospital discharge. Here, information is also displayed about the patient’s socio-medical situation, ADL-ability according to Katz, etc.

Minutes

In the contact between the different regional care providers it is often necessary to produce minutes of meetings to document discussions and decisions regarding patient activities. With the co-ordinatedl care planning module, this feature is readily available.

Integration with admission / discharge

The co-ordinated care planning module is tightly integrated with admission/discharge, and automatically links to information about discharge, care unit, treatment summary etc. Thus, the information is only recorded once and the need for double efforts is eliminated.

Reimbursement information

Reimbursement information and allocation of payment for patient activities between the regional care organizations is handled by the co-ordinated care planning module. The system calculates when responsibility for payment is transferred and the corresponding billing information is generated automatically
